Apply for a Library Card

At the Library, present a picture ID reflecting a current River Forest address (such as a driver’s license or state ID) and current proof of residency in River Forest (utility bill, bank statement, vehicle registration, or postmarked envelope sent within the last three months).

Children and teens under the age of 14 must have their parents sign their application for a card and present proof of River Forest residency.

Your card can be processed on the spot. Your card must be renewed every 5 years, and can be used in all Metropolitan Library System libraries, as well as Chicago Public Library, Dominican and Concordia University libraries, and many other Illinois libraries. If you would like to apply for a library card online, fill out the form at the bottom of the page. Please note that we cannot process renewals online, or new cards for students living on campus at Concordia and Dominican Universities. We are happy to process your card in person!

Library Card Application Form

Please fill out this form to have your new library card created. When you come to pick up your new card, you will need to provide a photo ID and a second item that shows proof of your name and River Forest address (A utility bill is perfect!). You will be able to pick up your library card after 48 hours.

Statement of Responsibility for Applicant: I agree to be responsible for this card and all materials checked out on this card until it is reported lost or missing. I am further responsible for any changes that may result from late return, loss, or damage of materials borrowed. I agree to comply with Library and System regulations.
